I frequently get asked, “Do I need to change my name when I get married?” In simple form, within New Zealand it is not necessary to officially change your name by Statutory Declaration, this would only be required if you were to adopt your maiden name or the like as your middle name. When you get married the assumption is made that you are about to do one of the following things :

Assume the surname of your partner
Continue with your current surname
Double barrel the surnames

Once married you can simply go about starting to use your assumed name. Different organisations of course have different rules for proving ID, generally speaking the best idea is to make sure you obtain a marriage certificate as proof to show you’ve taken on your partner's last name. On your wedding day you keep a copy of the particulars of marriage, simply fill out your details on the back and post your copy in to receive your typed marriage certificate. The cost is $26.50 to obtain your official marriage certificate. Alternativelyyou can order a marriage certificate over the phone, paying by either credit or debit card or through your Real Me verified online account.

A further option is to order via post; download the appropriate request form (see link below) and mail to Births, Deaths and Marriages. For this post option, you’ll need a referee to sign the form — they must be over 16 and have known you for at least 6 months or seen your official ID.
